What is Advanced Care Planning?

Advance Care Planning involves taking steps to carefully consider and plan for your future healthcare. It is a way to let your family and doctors know how you want to be cared for if you become unable to make decisions for yourself. It is about planning for a time when you may be seriously ill or injured so that your choices and preferences about personal care and medical treatment are clearly known to your family, doctors and carers. Then if you experience a serious deterioration in health or suffer a medical crisis, your Advance Care Plan can be activated and your wishes can be followed by everyone involved.
